Essential Lifestyle Plan (ELP)

Essential Lifestyle Plans (ELP) are a guided method for learning how someone wants to live and for developing a plan to help make it happen. This kind of planning is done with someone who listens very carefully to you, your family, and those who care for you. This method comes up with lists of what is important to you and gives information about getting supports. An ELP helps you plan the kind of life you want and pays special attention to your health and safety.


Making Action Plans (MAPS)

Making Action Plans (MAPS) uses your history, dreams, fears, and needs to write and draw a "map" of what you must do to gain the life you want. From your detailed answers to questions, you and your supporters can make an action plan that uses your gifts and abilities. Your supporters describe you with simple words so you can hear your strengths and learn where you need to get stronger. Then, you all design actions and make commitments that will help you reach your dream.


Personal Futures Planning

Personal Futures Planning uses pictures and signs to draw up a plan of the future you want. From those ideas, you can decide what activities and supports you need to move towards that future. People who were important in your past and now meet with you to share information. Then, you work together to develop a plan and commit to actions that will lead to a meaningful future.


Planning Alternative Tomorrows with Hope (PATH)

Planning Alternative Tomorrows with Hope (PATH) helps a group or an individual move towards developing a positive and possible future. You and your supporters begin to dream together to develop a clear idea of a future that you can reach on a realistic schedule. A facilitator guides the process and uses pictures and charts to help you come up with the supports and resources that you need to reach that goal. At the end of the process, you should have a detailed picture, specific steps, and a time line for moving forward with your dream.
